RESA 7 Assistive Device Depository
This project was funded by the West Virginia Department of Education, Office of Special Programs, Preschool/School Age, IDEA, Part VI B, and Discretionary Funds.
The purpose of this project allows county personnel in RESA 7 to borrow equipment/materials to use on a sixty day (60) trial basis. The equipment/materials are to be used for a student with an IEP that designates the use of a specific type of equipment. The use of this equipment on a temporary basis would ensure that said equipment/materials meet the student’s needs before being purchased.
An Assistive Device Catalog and Assistive Device Request Form is available on the RESA 7 website and also at the County Special Education Director’s Office.
You may visit RESA 7: Special Education: Forms and print off copies of the Catalog and Request Form. The Catalog has been coded for easy use:
CD – Communication Disorders
HI/APD – Hearing Impaired & Auditory Processing Disorders
VI – Visually Impaired
LD – Learning Disabled
OT/PT – Occupational Therapy & Physical Therapy
As new equipment is purchased, up-dates will be made to the online catalog. The request form MUST be completely filled in (including tag numbers), and the signatures of the School Principal and County Special Education Director before the request will be filled. It is your responsibility to pick up and return the items in a timely manner.
We hope that you will take advantage of the RESA 7 Assistive Device Depository. While devices are categorized under specific disciplines they may be used by students with various exceptionalities. The equipment/materials are costly and some are very delicate in nature. Therefore, when using borrowed items; we ask that you take care in the use and return of those items. If you are not familiar with the equipment, please request training from RESA trained personnel or possibly from another individual in your county who has already received training on the appropriate use of the request equipment/materials. Any lost or damaged items, while in use or in route to RESA 7 will be replaced or repaired by the borrowing county. NOTE: Certain equipment requires evaluations by trained professionals (e.g. audiologist) and recommendation by the IEP committee before being used for students. Special training and/or use under supervision of one who has received training is required for certain pieces of equipment. Please check the catalog for equipment with special requirements (**).

I understand that RESA 7 assumes no liability for the inappropriate or misuse of this equipment, resulting in injury to student(s), with the use of these devices. Any liability will be the sole responsibility of the borrowing county. Any equipment damaged or lost, while in use or in route to RESA 7, must be replaced by the county.
